Frame Boutique is an Out-Of-Network provider for the following vision insurance companies: The general procedure for reimbursement for these Insurance Providers is as follows:
  1. Fill out an Out-Of-Network Reimbursement Form.
  2. Include with the form an itemized receipt (Frame Boutique provides these with every purchase).
  3. Mail, Fax, or Email the completed form to the corresponding vision insurance provider.
